    Meeting Reservation Form

    Please complete a separate Reservation Form for each Member or Guest

    Reservations must be made at least 2 business days in advance. Cancellations must be made via email to cbelser@hirelevel.com at least one business day before the meeting, or a cancellation fee equivalent to the basic member meal fee will be assessed. This applies to all members and guests as the Chapter is responsible for the cost of the reserved meals.
